Our office environment is specifically designed to accommodate the comfort of parents and their children within a high-functioning medical facility. The office features clean, calming interiors and our sick and well waiting rooms are separated to maintain the health of our patients and their families.
In addition to bringing your child in when they are ill or need an examination, routine well care visits give us the opportunity to access the unique “big picture” version of your child’s overall health and wellness – information that helps us make the best choices about acute symptoms and treatment.
Our relationships with patients and their families are uniquely strong thanks to the importance our practice places on active listening, and we supplement this vital connection with confidential electronic medical records that allow us to process all the details of your child’s care.

Please note that you can either print these forms out and fill them in with a pen, or you can open the PDF and type in the information – at which point you must print the completed form and bring it into the office. We do not support the digital submission of forms at this time.
We participate with many insurance companies. Please call our office at 586.749.4444 for a complete list.
All patients must present their plan identification cards at each office visit, and co-payments and charges for non-covered services must be paid at each visit.
For patients without insurance, Tender Care Pediatrics requires complete payment for our services at the time of the office visit. We understand that this may be difficult, and are happy to discuss payment arrangements with you.
We accept cash, personal checks, Discover, MasterCard, Visa and American Express.
